What should I bring to my appointment?

If you are a new patient, please bring identification, insurance cards, medication list, and medical records. If you are a established patient please always bring your insurance cards and notify us of any changes in your insurance policy, phone, or address.

When should I call for a refill of my prescription?

Please call your pharmacy before your prescription runs out. They will fax us a request and we will return their request within 48 hours.

Do I need to fast?

Please do not fast for the following procedures:

PSA blood draw
Prostate Needle Biopsy
Cystoscopy
Vasectomy
